Oftedahl, Wisconsin land Surveyor, S-2594, do hereby <br />swear or affirm that a certain document which was titled as follows: <br /> <br />Certified Survey Map Number 12410, recorded on the 10th day of <br />March, 2008 as Document Number 4406618 in Volume 77 of Certified <br />Survey Maps on pages 176-178 and was recorded in Dane County, State <br />of Wisconsin, contained the following errors: <br /> <br />1) The monument marking the southwest comer of Lot 1 of the <br />above referenced Certified Survey Map is shown and noted as <br />being a 3/4- rebar set. <br />2) The legend notes monuments set as being u3/4D REBAR SEr. <br /> <br />AFFIANT makes this Affidavit for the purpose of correcting the above <br />document as follows: <br /> <br />1) The monument marking the southwest comer of Lot 1 of the <br />above referenced Certified Survey Map is a 2- diameter Pinch Top <br />Iron Pipe found. <br />2) The monuments set are in fact 314- x 18- REBAR SET WEIGHING <br />1.50 Lbs/LF. <br /> <br />AFFIANT is the drafter of the document being corrected. <br />A copy of Sheet 1 of the original document is attached. <br />Dated: i ~ \....
