ZUZ� <br /> EFFECTIVE DATE OF PERMIT 8/12/03 EXPIRATION DATE: ((See below) <br /> THE CONDITIONAL USE SHALL BE LOCATED ON THE PROPERTY DESCRIBED AS <br /> FOLLOWS: <br /> Part of the NW 1/4 NE 1/4 of Section 11, Town of Springfield <br /> described as follows : The North 200 feet of the East 300 feet of <br /> the NW 1/4 NE 1/4 of Section 11 . <br /> CONDITIONS: <br /> These conditions are specifically made applicable to this permit, <br /> any violation of such conditions shall be sufficient grounds for <br /> revocation of this permit. <br /> 1). The applicant shall submit an erosion control plan covering the entire CUP <br /> area for the duration of operations, and receive approval of an erosion control <br /> permit prior to commencing extraction operations. <br /> 2). The applicant shall apply for and receive all other required local, state and <br /> federal permits. <br /> 3). The operator shall develop and operate the site according to the <br /> site/operations and phasing plan dated 6/12/03. <br /> 4). Operations shall cease no later than 23 years from the date of CUP <br /> approval. <br /> Reclamation shall meet requirements of Chapter 74 of the Dane County <br /> ode of Ordinances. euJ-ze,rl-se 4t/L.J (i ) p.cic _ -,&. iO( t,)(f;) <br /> 6). The operator and all haulers shall access the CUP site only through those <br /> points designated as entrances on the Site/Operations Plan dated 6/12/03. <br /> 7). Hours of operation are from 6:00 a.m. to 9:00 p.m., Monday through <br /> Friday, and 8:00 a.m. to 12:00 p.m. on Saturday. <br /> 8). There shall be no blasting or drilling on the site. <br />
