The Department's review is based on the examination of the "Soil and Site <br /> Evaluation Report" as submitted by the Certified Soil Tester, Paul Hardy, CST # 225394, and the <br /> results of an onsite inspection by the Department on January 9, 2015. Based on these <br /> examinations, it is the opinion of the Department that the areas inspected appear to meet the <br /> minimum requirements of Wisconsin Administrative Code ch COMM 83 for the installation of the <br /> following type(s)of system(s): <br /> One soil boring verifies that existing drainfield is located in acceptable soil conditions. Owner is in <br /> process of obtaining permission for additon to house. A variance is necessary from State of WI, Dept <br /> of Safety&Professional Services to allow the septic tank to remain less than 5 feet from the proposed <br /> addition. Also, if the septic tank and soil absorption drainfield are to remain as currently located,the <br /> water well (under the house)will have to be properly abandoned by a licensed contractor and a new <br /> well would need to be constructed east or south of the house. <br /> Page 1 <br />
