The Department's review is based upon <br /> examination of(1)the"Soil and Site Evaluation report"as submitted by the Certified Soil <br /> Tester, P Q&' - y41,02 O y CST# zzs..394.and(2)the results of <br /> an en-site inspection by the Department. Based upon such examination,it is the opinion of the <br /> Department that the exact areas inspected appear to meet the minimum requirements of <br /> Wi$consin Administrative Code ch COMM 83 for the installation of: <br /> Conventional Soil Absorptioh System <br /> In-Ground Pressure Distribution System <br /> W.-Mound System <br /> ._ At-Grade Mound System <br /> Holding Tank <br /> Other: <br /> The above opinion is advisory only. Any disturbance to the exact area that was investigated, <br /> including compaction,excavation and filling or removal of soil,will invalidate this opinion.
