The Department's review is based on the examination of the "Soil and Site <br /> Evaluation Report" as submitted by the Certified Soil Tester, Steve Crosby, CST # 227009, and <br /> the results of an onsite inspection by the Department on (Missing Soil Test Date). Based on these <br /> examinations, it is the opinion of the Department that the areas inspected appear to meet the <br /> minimum requirements of Wisconsin Administrative Code ch COMM 83 for the installation of the <br /> following type(s) of system(s): <br /> Conventional System onsited in 2003. <br /> The final system design must comply with all State and County code criteria, including a <br /> minimum of three soil pits per system area. <br /> The above opinion is advisory only. Any disturbance to the area investigated, including <br /> compaction, excavation, refilling, or removal of soil, will invalidate this opinion.
