The applicant shall modify the gravel areas to bituminous, or Portland <br /> Cement concrete in accordance with City of Madison standards and <br /> specifications. All off-street facilities shall be paved in accordance to City <br /> of Madison General Ordinance Section 10.08(6)(a)10. <br /> 5. The Traffic Engineer may have additional comments and/or requirements <br /> upon review of the final building plans and site plans. This proposed project <br /> shall comply with all applicable Madison General Ordinances and policies. <br /> Please contact Gary Dallmann, City Engineering at 266-4751 if you have <br /> questions regarding the following item: <br /> 6. The applicant shall be responsible for obtaining all necessary permits issued <br /> by other municipalities and shall be responsible for complying with any <br /> requirements imposed by other jurisdictions. If any of the requirements <br /> imposed by any other jurisdiction conflicts with a plan approved by the City <br /> of Madison, it shall be the applicant's responsibility to resolve such conflicts <br /> and submit a revised plan to the City of Madison. <br /> Please contact me or Brad Murphy of the Planning Unit staff at 266-4635 if <br /> you have questions regarding the following two items: <br /> 7. The applicant shall furnish a written agreement providing that the applicant <br /> shall not prevent or deny space on this tower or site for other carriers and <br /> shall make this facility available to share at fair market rate as determined <br /> by customary industry standards. <br /> 8. The applicant shall provide a written agreement stating that if the tower or <br /> antennas or transmitters are unused for a period exceeding 12 months the <br /> applicant shall remove the tower antennas or transmitters upon written <br /> request from the City of Madison Building Inspection Unit Director at no <br /> cost to the City or the property owner within 30 days of said request. <br /> Please contact Bill Bakken of the Inspection Unit at 266-4551 if you have <br /> questions on the following item: <br /> 9. The tower must be reviewed by the City Inspection Unit staff to ensure that <br /> the tower can accommodate multiple users. <br /> Please follow the procedures listed below to receive your conditional use <br /> approval. <br /> 1. Please revise plans per the above conditions and submit five (5) sets of the <br /> final site plans to the Zoning Administrator.
