CONDITIONAL USE PERMIT# 1519 by Middleton Community Church to allow religious uses-church on property located North of 3978 Schewe Road in part of the NE 1/4 NE 1/4 Section 20,and NW 1/4 NW 1/4 Section 21,Town of Middleton. Motion by Hendrick/Comwell to impose the following conditions: 1.Dedication of an additional 7 feet of road right-of-way on Old Sauk Road. 2. Vehicular access to Old Sauk Road is prohibited. 3.Building permit issuance is subject to site approval by the Town Board based upon recommendation of the Town eingineer and Town administrator. 4.The area east of the building envelope shall be maintained as open space planted in native grasses and wild flowers.No development shall be allowed in this area without the prior written approval of the Town Board. 5. The petitioner shall comply with the Town's landscape ordinance. 6. Construction shall not commence until the Town engineer is satisfied that the vision corner at Schewe Road and Old Sauk Roos is satisfactory. 7.The church must submit a"lighting plan"for approval by the Town Board. 8.The church must submit a manure management plan regarding the agricultural activities which will continue on the property for approval by the Town Board. 9. Submittal of a site plan to the Planning and Development Department. Motion carried 5-0. Motion by Hendrick/Cornwell to find that the standards of the zoning ordinance are met and to approve the Conditional Use Permit as conditioned;carried 5-0. 49. Consideration of proposed amendments to the Dane County Farmland Preservation Plan relating to plan amendments in the Town of Blue Mounds. -Approved at previous meeting. 50. CUP 1511/7292,Gyland,Medina,7/28/98 Motion by Hendrick/Anderson to impose the following conditions: 1.That the C-1 rezone area be utilized only for a veterinary clinic with dog and cat boarding and grooming and training facility. 2.That the boarding kennel and groomimg salon be for 100 dogs and cats only. 3. Submittal of an acceptable manure storage plan.
