SPILL INFORMATION <br /> When you report a spill to company or to State or local government <br /> personnel , you may need to provide the following information about <br /> the spill : <br /> 1 . Name, address and phone number of the person <br /> reporting the spill ; <br /> 2 . Name, address and phone number of the person who <br /> caused the spill ; <br /> 3 . Date, time, location and duration of the discharge <br /> that caused the spill ; <br /> 4 . Date and time that the notification is being made; <br /> 5 . Identity, quantity and physical state (solid, liquid <br /> or gas) of the discharged material; <br /> 6 . Physical, chemical, hazardous and toxicological <br /> characteristics of the discharged material . This <br /> information is usually on the Material Safety Data <br /> Sheet (MSDS) that the company will have on file for <br /> the material . You can either find an MSDS in your <br /> right-to-know binder, or one can be located at the <br /> human resources department in the Waukesha office of <br /> P&D, at (414) 524-1700 . <br /> 7 . Cause of the spill and the corrective measures being <br /> taken; <br /> 8 . Source, speed of movement and destination or <br /> probable destination of the spilled substance; <br /> 9 . Distance and direction to the nearest inhabited <br /> dwellings; <br /> 10 . Areas that may be affected by the spill , including <br /> air, surface, ground or drinking water; <br /> 11 . Weather conditions at the site of the spill ; <br /> 12 . Communications equipment at the site . <br />
