suaM ire ay <br /> APL (Ari <br /> zI S/yg <br /> PROPOSED CONDITIONS FOR CUP 1476 <br /> 1. An Erosion Control Plan and Permit will be in place before mineral extraction takes place. <br /> 2. Town of Cottage Grove conditions of approval to be followed(listed below). <br /> a. Off-site removal of overburden(clay and topsoil)not be allowed. <br /> b. Trucks should exit the site directly to the 12/18 construction rather than CTI-I <br /> `N' whenever possible and applicants should work with the DOT and <br /> contractors to accomplish this goal. <br /> c. Hauling from the site should not begin until the 12/18 construction begins and <br /> reclamation for agriculture use must be completed within 5 years. <br /> d. Town will review site conditions when renewing annual permits. <br /> 3. Initial site development shall include the following: <br /> a. Pertinent erosion control measures in place(i.e.silt fence, detention basins, <br /> diversion berms/ditching,etc.) <br /> b. Stop sign and gated entrance in place. <br /> c. First 150 feet of access paved asphalt with the following 400 feet graveled. <br /> d. Establish as soon as possible a screening berm, 10 feet high, along the entrance <br /> as shown on the revised site plan. The berm will be seeded. <br /> c. Place orange safety fencing along a line parallel to and 300 feet from the bank of <br /> Little Door Creek. <br /> 4. Dust and noise mitigation will be addressed as follows: <br /> a. Mechanical spraying of the site will take place as needed. <br /> b. Spray bars can be incorporated on crushing equipment as needed. <br /> c. Sound suppressing equipment will be required on all equipment and trucks. <br /> d. Access road will be paved and graveled as outlined above. <br /> e. Berming will be established per erosion control plan and site plan. <br /> 5. Hours of operation: <br /> a. General mineral extraction site hours will be 6:00 am to 6:00 p.m. Monday thru <br /> Friday, and 6:00 am to 3:00 p.m. Saturday. <br /> b. Crushing,screening and washing hours will be 8:00 am to 5:00 p.m. Monday <br /> thru Friday, and 8:00 am to 3:00 p.m. on Saturday. <br /> 6. The planed time frame of the mineral extraction is for 4 years with reclamation taking place by the <br /> completion of year five. <br /> 7. Due to the relatively short time frame,the mineral extraction will be phased in two parts(three if <br /> you include final reclamation). See revised phasing operations plans. <br /> 8. No excavation below ground water is expected(test holes were dug to—25 feet deep with no <br /> signs of ground water). Water used for washing and dust control would come from wells or <br /> pumped from the creek(with proper DNR permits). <br /> 9. The Dane County Highway Department has determined that we should use the existing field <br /> entrance onto Hwy N or close to this. They also determined that for now no improvements will be <br /> needed but that they may determine at a latter date that due to traffic an accel/decel lane may be <br /> needed along the west shoulder of Hwy N. <br /> 10. Revised site plan,erosion control plan,operations phasing plans,and reclamation plan have been <br /> submitted showing the above mentioned changes where applicable. <br /> 11. The site will be protected from the public by the following: <br /> a. Little Door Creek to the north and west. <br /> b. New proposed Hwy 12/18 right-of-way fencing to the south and along portions <br /> of the site to the east(southern part). <br /> c. Screening berm and diversion berm/ditching for the remainder of the eastern site <br /> boundary. <br /> d. Note that there will also be orange safety fencing placed parallel and 300 feet to <br />• the south of the creek. <br /> - 12. All equipment will be stored or placed above ground water. Most equipment will be staged by a <br /> portable scale and trailer located at the end of the graveled access road. <br /> 13. Post "No Trespassing"signs around site. <br />
