Dvorak and Laurie J. Dvorak, on oath depose and say that: <br /> 1. On October 8, 1996 Affiants executed,along with Gregory Shaw,Diane S.Shaw <br /> and David W. Bisbee, as owners of said lands described above, a Petition to Amend Zoning <br /> Amendment No. 4520 and 6175 (revised 9/25/96) ("Petition"). Attached is a copy of said <br /> Petition. <br /> 2. Attached to that Petition were certain restrictions("Restrictions"). Attached to <br /> the Petition is a copy of said Restrictions. Said Restrictions were recorded at the Dane <br /> County Register of Deeds on January 27, 1997 as Document No. 2828625. <br /> 3. From the Petition, Dane County Zoning Amendment No. 6687 was duly <br /> adopted by the Dane County Board of Supervisors on November 7, 1996 and approved by the <br /> County Executive on November 11, 1996. Attached is a copy of said Dane County Zoning <br /> Amendment No. 6687. <br /> 4. Dane County Ordinance Amendment No. 6687, at paragraph B. under Deed <br /> Restriction Required contains certain language that was to have been included in the <br /> Restrictions, but was inadvertently omitted.
