CUP 1156-C PAGE 3 of 4 <br /> CONDITIONS <br /> These conditions are specifically made applicable to this permit, <br /> any violation of such conditions shall be sufficient grounds for <br /> revocation of this permit. <br /> 1. The conditional use permit shall be limited to the <br /> parcels of land described in the permit application. <br /> 2. The permit shall be effective only for so long ass the <br /> land in question is owned by. the Wisconsin Heights School District <br /> or a suOceesor public school agency. <br /> 3 . The zoning of all land subject to the permit shall be <br /> Exclusive Agricultural; if it needs to be rezoned to be so zoned, <br /> then said rezoning is a condition of the permit. <br /> 4. The Town Board shall be presented with the site <br /> evaluation for on-site private sewage treatment plans for the <br /> school(B) . and additional unsewered facilities a3 they are <br /> constructed. The evaluation shall determine whether a septic tank, <br /> mound system, alternative system or holding tank is appropriate to <br /> handle the on-aite private sewage treatment for the school <br /> facilities. • <br /> 5. If a holding tank is used for any on-site private sewage <br /> disposal, the Wisconsin Heights School District shall submit an <br /> application to the Town for a holding tank permit and shall provide <br /> evidence of its fiscal resp-oneibility and pumping arrangements for <br /> the tank(s) . <br /> 6. The Wisconsin Heights School Distriot shah prepare a <br /> detailed stormwater ma.nagetent control plan for the entire school <br /> property, as well as any future expansions of the property, showing <br /> how the District will prevent adverse impacts from stormwater <br /> .runoff. If the plan indicates that a detention basin or other <br /> rU4dff control facilities are needed, the construction of those <br /> control facilities shall be a condition of this permit. Particular <br /> attention shall be given to the need to protect the slack Earth <br /> Creek and its watershed from runoff. <br /> 7. The Wisconsin Heights School District shall also prepare <br /> a detailed erosion control plan for the construction phaeo of the <br /> building program and for the operation of the facilities after <br /> construction. Particular attention shall be given to the need to <br /> protect the Black Earth Creek and its watershed from runoff. <br /> 8. There shall be a study of the impacts of the school <br /> building program on the transportation facilities in the vicinity <br /> of the school, including but not limited to: <br /> • <br /> A. Whether turning/deceleration/acceleration lanes should be <br /> constructed on U.S, Highway 14 at the school grounds entrances and <br /> exits. <br /> 8, Whether railroad crossing wig-wag signals should be <br /> installed on Olson Road where the railroad crosses that road. <br /> PAGE 3 of 4 <br />
