Dane County Planning & Development <br />Division of Zoning, (608) 266-4266 <br />***NOTICE: INFORMATION FOR THOSE OBTAINING <br />EROSION CONTROUSTORMWATER MGMT PERMITS*** <br />Please read over your approval letter from Land Conservation and the <br />conditions indicated on it. If you see any problems or do not understand it, <br />please contact the Land Conservation staff member who wrote it <br />immediately, at (608) 224-3730. You will be held responsible for meeting all <br />of the conditions and timelines unless you are granted written revisions. <br />Your Dane County Erosion Control/Stormwater Management/ <br />Redevelopment Permit under Chapter 11 or 14 of the Dane County <br />Ordinance will expire on the stabilization date noted in your approval letter <br />from the Dane County Land Conservation Division. These permits can only <br />be revised or extended with the written approval of D.C. Land Conservation <br />before the stabilization date. <br />To apply for an extension, you must contact D.C. Land Conservation in <br />writing before the stabilization date and they must acknowledge receipt of <br />the request, and then issue written approval with copies to Dane County <br />Zoning. <br />Once a permit expires, it is considered null and void, and if the site has not <br />been stabilized to Dane County standards, you will be required to cease all <br />work on the property, apply for a new permit in complete form, pay <br />applicable fees, and wait the 15 or more working day waiting period for <br />approval of the new permit, as well as face possible citations and fines for <br />each day the site is in violation. <br />If you need assistance, you may contact: <br />Jess Starks- Dane County Zoning <br />(608) 266-1591 <br />starks@co.dane.wi.us <br />Ms. Starks is our Erosion Control & Stormwater Specialist, and she may be <br />able to further explain details of the timing of your permit. <br />STATEMENT TO READ AND SIGN AFTER APPROVAL LETTER <br />RECEIVED, AND BEFORE PERMIT CARD ISSUED: <br />Re: Permit #: <br />I, (Printed Name: -1T r 1< c ; .; ) have read, <br />understand, and will take responsibility as owner, applicant, or <br />agent, for all points addressed in the above notice. <br />Signed: �. _----____ � Date: <br />�" kOriginal- to Jess for Permit File, Yellow- to Applicant, Pink- to Owner <br />
