elevations and delineated wetland boundaries,if any. If not available, appropriate flood zone determination <br /> and/or wetland delineation may be required at the applicant's expense; <br /> 5. The extent and location of all soil types as described in the Dane County Soil Survey,slopes exceeding. <br /> 12%,and areas of natural woodland or prairie; <br /> 6. The limits of construction,filling,grading and/or other activities requiring a grading and filling permit. <br /> including the area in square feet; <br /> 7. Existing and proposed elevations(referenced to the National Geodetic Vertical Datum of 1929)and existing <br /> and proposed contours in the area requiring a grading and filling permit; <br /> 8. Elevations,sections,profiles,and details as needed to describe all natural and artificial features of the <br /> project; <br /> 9. Soil erosion control and overland run-off control measures, including run-off calculations as appropriate; <br /> 10.Detailed construction schedule; <br /> 11.Copies of permits or permit applications required by any other governmental entities or agences. <br /> 12.Any other information necessary to reasonably determine the location,nature and condition of any physical <br /> or environmental features of the site. • <br /> (b) Where total disturbed area does not exceed 4,000 square feet,and where the zoning administrator <br /> determines that such information is not necessary to determine the. impact of the proposed project, the zoning <br /> administrator may waive requirements for signature by a professional engineer, run-off calculations or <br /> references to National Geodetic Vertical Datum. <br /> (c)The zoning administrator,or his or her designee, shall determine if an application is complete and adequate. <br /> A plan review period not to exceed ten(10)business days shall begin upon receipt of a complete grading and <br /> filling permit application,the application fee,and the required plans and specifications as listed above. <br /> (d)The zoning administrator,or his or her designee, shall send written notification of all grading and filling <br /> permit applications to the appropriate local office of the state Department of Natural Resources within three (3) <br /> business days of the date a complete application is received. <br /> (e)The zoning administrator,or his or her designee, shall send copies of approved or denied grading and filling <br /> permits to the appropriate local office of the state Department of Natural Resources within ten(10)business <br /> days of approval or denial. <br /> (5)Permit conditions: In considering a grading and filling permit,the zoning administrator shall evaluate the <br /> effect of the proposal as to possible water pollution including erosion and sedimentation,harmful changes to <br /> fish life or aquatic plants, and maintenance of safe and healthful conditions. <br /> (a)In granting a grading and filling permit,the zoning administrator shall attach the following conditions where <br /> appropriate: <br /> 1.The smallest amount of bare ground shall be exposed for as short a time as feasible. <br /> 2.Temporary ground cover(such as mulch)shall be used and permanent cover(seeding or sod) shall be <br /> planted. <br /> 3. Sediment shall be controlled according to best management practices recommended by the Dane County <br /> Land Conservation Department. <br /> 4.Lagooning shall be conducted in such a manner as to avoid creation of fish trap conditions. <br /> 5.Fill shall be stabilized according to accepted engineering standards. <br /> 6.Fill shall not restrict a floodway or destroy the storage capacity of a flood plain. <br /> 7.Fill shall not be deposited in a shoreland or inland wetland district, except in accordance with Sections 11.07, <br /> 11.08, 11.09 and 11.10 of this ordinance. <br /> 8. Stockpiling or temporary deposition of excavated materials shall not be permitted within the building setback <br /> area described in Section 11.03(2)of this ordinance. <br /> 9. Sides of a channel or artificial water course shall be stabilized to prevent slumping. <br /> 10.Sides of channels or artificial watercourses shall be constructed with side slopes of two(2)units horizontal <br /> distance to one(1)unit vertical or flatter,unless vegetative cover,bulkheads or rip-rapping are provided. <br />
