Pavlak, Helen D. Pavlak, Nolan B. <br /> Anderson and Susan Anderson. Susan Anderson is Ray and Helen Pavlak's <br /> daughter. <br /> Andersons are purchasing the subject property from Pavlaks on land <br /> contract. Therefore, for the purposes of this application, both Andersons <br /> and Pavlaks are identified as owners (collectively herein, "Owners"). <br /> Please use 763 CTH BB, Deerfield, WI 53531 as the address for the <br /> Owners and 849-5039 as the daytime telephone for the Owners. <br /> The subject property is described on the attached Certified Survey Map <br /> #7072 as Lot #1 ("Property"). The tax parcel # is 0712-103-9010-7. <br /> The Property is currently zoned RH-2 and covers approximately 6.8 acres. <br /> The Owners are requesting a Conditional Use Permit under RH-2 zoning to <br /> allow a dependency living arrangement. The Pavlaks are in their 80's and <br /> wish to be near their daughter and son-in-law without being a burden to <br /> them. Consequently, the Owners wish to design a home that has a separate <br /> living space, including separate kitchen and bath facilities, to allow the <br /> Pavlaks to continue to live independently. The home will be designed so <br /> it can easily be converted to a traditional single family dwelling (i.e., one <br /> without a separate living space) when the Pavlaks no longer live there.
