GENERAL-SURFACE AND UNDERGROUND (1) The employer shall establish a definite effective program <br /> and continually funcitoning safety program and make every attempt to prevent accidents and increase safety. Employes shall actively participate <br /> in the safety program. <br /> NOTE ITEM CODE <br /> SECTION <br /> Summary and Recommendations - Ahlgrimm Explosives Company's blastinc <br /> operations at Yahara Materials' Ziegler Quarry, Middleton, <br /> Wisconsin, were monitored. This monitoring was in response to at <br /> complaint of excessive ground vibration and possible structural <br /> damage filed by Mary Spitz, 6633 Greenbriar Road, Middleton, <br /> Wisconsin 53562. Vibration levels observed and estimated from <br /> blasters logs for prior work at this location appear to be in <br /> compliance with ILHR Chapter 7. -• <br /> - It is recommended that until further notice is given,--the Ahlgrimm <br /> Explosives Company furnish this department with adequate notice of <br /> blasting operations at the Ziegler quarry to allow -for department <br /> personnel to be present to monitor ground vibration levels at the <br /> Spitz residence. <br /> Complaint History - Mrs. Spitz's complaint was received by <br /> department personnel on April 24, 1989. Mrs. Spitz commented that <br /> the blast on April 17, 1989 was severe and similar to an earthquake, <br /> causing beams in her house to move as much as 6 inches. In <br /> addition, she indicated that neighbors had complained of similar <br /> severe vibrations. Mrs. Spitz also indicated that there had been <br /> additional blasting after April 17, 1989 shot, but they had not been <br /> as severe. <br /> On May 2, 1989 contact was made with Roger Lee, Yahara Materials, to <br /> discuss the Spitz complaint.
