DANE COUNTY REGIONAL AIRPORT <br /> 4000 International Lane Madison, WI 53704 P: 608 • 246 • 3380 F: 608 • 246 • 3385 msnairport.com <br /> /00•114 <br /> mi <br /> RECEIVED COUNTY <br /> May 6, 2016 <br /> REGIONAL <br /> AIRPORT <br /> Scott Schroeckenthaler MAY 9 2016 M A D I S O N <br /> Zoning Inspector <br /> Dane County Zoning Division <br /> 210 Martin Luther King Jr. Blvd. <br /> Department of Planning and Development DANE COUNTY PLANNING&DEVELOPMENT <br /> Room 116, City-County Building <br /> Madison, WI 53703-3342 <br /> Re: Permit Application under §78.06 Dane County Ordinance <br /> Monopole WI-00-1452 Madison Northport <br /> N43-07-51.34 W89-21-13.45 <br /> Dear Mr. Schroeckenthaler: <br /> Below are the conditions the Airport requests the Zoning Administrator establish for issuance <br /> of the permit. <br /> CONDITIONS APPLICABLE TO CRANE, MONOPOLE PERMIT: <br /> (1) Applicant shall comply with all conditions and recommendations set forth in <br /> the Determination Letter regarding Federal Aviation Administration Aeronautical Study No. <br /> 2015-AGL-3840-OE. <br /> (2) Prior to erecting the construction crane, applicant shall provide the on duty Airport <br /> Operations Supervisor, Dane County Regional Airport (office: 608.246.3397, cell: <br /> 608.235.1001), with the name and telephone number of an individual(s) having authority over <br /> operation and placement of the crane while it is on the construction site and, upon request, <br /> shall arrange a meeting between Airport staff and the contractor that will be operating the <br /> crane. <br /> (3) Applicant shall, at least three days before the crane height will reach 1,029 feet MSL, <br /> provide notice to Airport Operations. <br /> (4) At all times the highest point of the crane shall be marked during daylight hours with an <br /> orange and white checkered construction flag measuring at least three feet by three feet, and <br /> with a red obstruction light, with a minimum intensity of 32.5 candela, from two hours before <br /> sunset until two hours after sunrise and during periods of reduced visibility. <br />
