The Department's review is based on the examination of the "Soil and Site
Evaluation Report" as submitted by the Certified Soil Tester, Paul Hardy, CST #225394, and the
results of an onsite inspection by the Department on January 27, 2016. Based on these
examinations, it is the opinion of the Department that the areas inspected appear to meet the
minimum requirements of Wisconsin Administrative Code ch COMM 83 for the installation of the
following type(s) of system(s):
Soil Evaluation Report documents the fact that Holding Tank is the only available option for the
proposed building at this site due to site limitations.
The final system design must comply with all State and County code criteria, including a
minimum of three soil pits per system area.
The above opinion is advisory only. Any disturbance to the area investigated, including
compaction, excavation, refilling, or removal of soil, will invalidate this opinion. The Department
reserves the right to require a new soil morphological evaluation as may become necessary to
ensure compliance with all applicable codes prior to issuing a sanitary permit.
Sincerely,
Richard Herro, Sanitarian
Environmental Health Division
Page 1
