DANE COUNTY Page 1 of CONDITIONAL USE PERMIT #1000 THE ZONING AND NATURAL RESOURCES COMMITTEE OF THE DANE COUNTY BOARD PURSUANT TO SECTION 10.255 (2) OF THE DANE COUNTY CODE OF ORDINANCES DOES HEREBY: GRANT the conditional use permit subject to conditions. FOR: Day care center. EFFECTIVE DATE OF PERMIT: 6/23/92 EXPIRATION DATE : 6/23/97 THE CONDITIONAL USE SHALL BE LOCATED ON THE PROPERTY DESCRIBED AS FOLLOWS: Part of the SE 1/4 NE 1/4 Section 22, Town of Westport described as follows: Lot #1, CSM 3654 found in Volume 15, Page 43 of Certified Survey Maps Dane County Register of Deeds Office. CONDITIONS These conditions are specifically made applicable to this permit, any violation of such conditions shall be sufficient grounds for revocation of this permit. 1) . Maximum of 20 children. 2) . A minimum of 5 parking spaces shall be provided for clients . 3) . One sign is allowed, maximum of 4' x 5' in size and shall not be lighted. 4) . Permittee has agreed that, at the discretion of the Town of Westport or County of Dane, the permit may be subject to an annual review for each of the next three years . The purpose of said review shall be to examine unanticipated burdens that the facility may impose upon the community. At the time of the review, it shall be determined. a) . Whether any such burden can be ameliorated without further hearing. b) . Whether a full hearing in front of the Town of Westport Plan Commission is warranted. Permittee agrees that if such full hearing is held and it is the finding of the Town of Westport and County of Dane that further restrictions upon the operation of the day care are necessary and reasonable, such restrictions may be imposed by Dane County. Nothing contained in this condition shall allow the County of Dane to prohibit the original intended use so that the viability of th use is threatened. 5) . The permit shall expire 5 years from the date of issue.
