DANE COUNTY Page 1 of <br /> CONDITIONAL USE PERMIT #1000 <br /> THE ZONING AND NATURAL RESOURCES COMMITTEE OF THE DANE COUNTY BOARD <br /> PURSUANT TO SECTION 10.255 (2) OF THE DANE COUNTY CODE OF ORDINANCES <br /> DOES HEREBY: <br /> GRANT the conditional use permit subject to conditions. <br /> FOR: Day care center. <br /> EFFECTIVE DATE OF PERMIT: 6/23/92 EXPIRATION DATE : 6/23/97 <br /> THE CONDITIONAL USE SHALL BE LOCATED ON THE PROPERTY DESCRIBED AS <br /> FOLLOWS: <br /> Part of the SE 1/4 NE 1/4 Section 22, Town of Westport described as <br /> follows: Lot #1, CSM 3654 found in Volume 15, Page 43 of Certified <br /> Survey Maps Dane County Register of Deeds Office. <br /> CONDITIONS <br /> These conditions are specifically made applicable to this permit, <br /> any violation of such conditions shall be sufficient grounds for <br /> revocation of this permit. <br /> 1) . Maximum of 20 children. <br /> 2) . A minimum of 5 parking spaces shall be provided for clients . <br /> 3) . One sign is allowed, maximum of 4' x 5' in size and shall not <br /> be lighted. <br /> 4) . Permittee has agreed that, at the discretion of the Town of <br /> Westport or County of Dane, the permit may be subject to an annual <br /> review for each of the next three years . The purpose of said <br /> review shall be to examine unanticipated burdens that the facility <br /> may impose upon the community. At the time of the review, it shall <br /> be determined. <br /> a) . Whether any such burden can be ameliorated without <br /> further hearing. <br /> b) . Whether a full hearing in front of the Town of Westport <br /> Plan Commission is warranted. Permittee agrees that if such <br /> full hearing is held and it is the finding of the Town of <br /> Westport and County of Dane that further restrictions upon the <br /> operation of the day care are necessary and reasonable, such <br /> restrictions may be imposed by Dane County. Nothing contained <br /> in this condition shall allow the County of Dane to prohibit <br /> the original intended use so that the viability of th use is <br /> threatened. <br /> 5) . The permit shall expire 5 years from the date of issue. <br />
