AIR POLLUTION CONTROL PERMIT APPLICATION <br /> FORM 4500-1T: STORAGE TANKS <br /> Complete one form for each storage tank for which an Air Pollution Control Permit is required. <br /> Item 1 — Provide the facility name. <br /> Item 2 — Assign an identification number to this storage tank (e.g., T-1, T-2, etc.). This number should <br /> appear on the appropriate Form 4500-1C: Air Pollution Control Equipment completed for this <br /> storage tank. <br /> Item 3 — Provide an identification number for the control device, if any, for this storage tank. This number <br /> should also appear on the appropriate Form 4500-1C: Air Pollution Control Equipment completed <br /> for this control device. <br /> Item 4 — Self explanatory. <br /> Item 5 — Self explanatory. <br /> Item 6 — If the tank roof is sloped, provide the average tank height. <br /> Item 7 — Self explanatory. <br /> Item 8 — Self explanatory. <br /> Item 9 — A submerged fill pipe is any fill pipe with a discharge opening which is entirely submerged when <br /> the liquid level is six inches (15.2 centimeters) above the tank bottom. <br /> Item 10 — Check the appropriate emission control equipment, if any. Specify the vapor recovery system, if <br /> used (e.g., vapor/liquid absorption, vapor compression, vapor refrigeration, vapor/solid absorption, <br /> vapor balance, etc.). <br /> Item 11 — Self explanatory. <br /> Item 12 — Complete this table for all materials which are stored in this tank. Vapor pressures should be given <br /> as real vapor pressures at tank conditions given. Do not supply Reid vapor pressure. <br /> Item 13 — Also indicate here if this tank will serve operations at: a bulk gasoline terminal which receives <br /> gasoline from refineries, a bulk gasoline plant which receives gasoline from bulk gasoline terminals <br /> for subsequent distribution to dispensing facilities, or a pharmaceutical manufacturing facility. <br />
