Dane County Land&Water Resources Department <br /> ,p s# Jeremy Balousek,P.E.Division Manager <br /> C014 <br /> INFORMATION FOR THOSE OBTAINING EROSION CONTROL <br /> & STORMWATER MANAGEMENT PERMITS <br /> Read the permit approval letter and understand the conditions under which <br /> the permit was approved. <br /> The permit will expire on the stabilization date shown on this permit card. <br /> The permit may only be revised or extended prior to the expiration date with <br /> the written approval of Dane County Water Resource Engineering. <br /> Once a permit expires it is null and void. If the site has not been stabilized by <br /> the expiration date a new permit will be required. Double fees will be charged <br /> for all remaining disturbed areas. <br /> The failure to comply with the conditions of the permit will be considered a <br /> violation of County Ordinance and may result in a stop-work order, citations <br /> and fines for each day the site is in violation. <br /> The permit is for land disturbing activities regulated under Dane County <br /> Chapters 11 or 14 only. Other municipal, county, state or federal permits may <br /> be required. <br /> As-built certification for Stormwater Management Permits must be <br /> submitted to the Water Resource Engineering Division after site <br /> stabilization.
