y Page 1 DANE COUNTY CONDITIONAL USE PERMIT #1510 THE ZONING AND NATURAL RESOURCES COMMITTEE OF THE DANE COUNTY BOARD PURSUANT TO SECTION 10.255 (2) OF THE DANE COUNTY CODE OF ORDINANCES DOES HEREBY: GRANT: The Conditional Use Permit subject to conditions. FOR: Television tower. EFFECTIVE DATE OF PERMIT: 7/28/98 EXPIRATION DATE: See Below THE CONDITIONAL USE SHALL BE LOCATED ON THE PROPERTY DESCRIBED AS FOLLOWS: Part of the SE 1/4 SW 1/4 of Section 35, Town of Madison described as follows: Lot #2 CSM #8784 found in Volume48, Page 279 of Certified Survey Maps Dane County Register of Deeds Office. CONDITIONS: These conditions are specifically made applicable to this permit, any violation of such conditions shall be sufficient grounds for revocation of this permit. 1. The primary use of the communication tower permitted under Conditional Use Permit No. 1510 shall be for the mounting of equipment for Personal Communication Services (PCS) transmissions/receptions/receptions and Cellular Telephone equipment. 2. The communication tower placed, constructed or modified under Conditional Use Permit No.1510 shall accommodate, at a minimum height of 180 feet above grade, the collocation of two additional antenna arrays similar in size and function to that placed on the tower by the holder of said permit. 3. The two collocation sites required hereunder need not be available on the tower as initially placed, constructed, or modified, provided that the tower will support at the specified minimum height the later addition of the required number of collocation sites. 4. Collocation sites required hereunder shall upon request be made available by the holder of Conditional Use Permit No. 1510 for the mounting of technologically compatible antenna arrays and equipment at the prevailing market rate in the region and upon contractual provisions which are standard in the industry.
