FOR: Communications tower site. 12/12/00 (amended legal) EFFECTIVE DATE OF PERMIT: 9/12/00 EXPIRATION DATE: (See Below) THE CONDITIONAL USE SHALL BE LOCATED ON THE PROPERTY DESCRIBED AS FOLLOWS: A part of the NW 1/4 NE 1/4 of Section 4, Town of Deerfield described as follows : Commencing at the North quarter corner of said Section 4; thence N87 °54' 58" East 12 . 90 feet along the North line of the NE 1/4 of said Section 4, thence S03°15' 33" East 236 . 45 feet to the South right-of-way line of Interstate #94; thence N73°48' 27" East 242 . 18 feet; along said South line; thence Easterly 147 . 47 feet along said South line and the arc of a curve to the right, having a radius of 1317 . 69 feet, and a chord of which bears N76°20' 49" East 147 . 47 feet; thence S03°15' 33" East 37 . 66 feet to the point of beginning; thence S86°44' 27" West 100 . 00 feet; thence S03°15' 33" East 100 . 00 feet; thence N86°44' 27" East 100 . 00 feet; thence NO3°15' 33" West 100 . 00 feet to the point of beginning. CONDITIONS : These conditions are specifically made applicable to this permit, any violation of such conditions shall be sufficient grounds for revocation of this permit . 1 . The primary use permitted under Conditional Use Permit No. 1559 shall be for the construction of a 200' communication tower and the mounting of U. S. Cellular communication equipment . 2 . The communication tower constructed or modified under Conditional Use Permit No. 1559 shall accommodate, at a minimum height of 150 feet above grade, the collocation of five additional antenna arrays similar in size and function to that placed on the tower by the holder of said permit .
