NEVER. ENTER A TREATMENT TANK OR HOLDING TANK UNDER ANY
CIRCUMSTANCE. DEATH MAY RESULT. ESCAPE OR RESCUE FROM THE INTERIOR OF A TANK IS VERY
?...4 DIFFICULT.
• ADDITIONAL INFORMATION:
POWTS INSTALLER POWTS MAINTAINER
IName: Leyake So;I Jrsrn PLC Name:
Phone: 0 R $• 7-z" I Phone:
•
SEPTAGE•SERVICING OPERATOR(PUMPER) LOCAL REGULATORY AUTHORITY
`Nom: I Name:
Phone: Phone: h baB•ZN Z't<S to
ThIs document Is Intended to meet minimum requirements of Ch.Comm 83.22(2)(bX1Xd)&(f)and 83.54(1),(2)&(3),Wisconsin Administrative Code,
Use of this document does not guarantee the performance of the POWTS.
}
.dea •
