Page 1 DANE COUNTY CONDITIONAL USE PERMIT #1400 THE ZONING AND NATURAL RESOURCES COMMITTEE OF THE DANE COUNTY BOARD PURSUANT TO SECTION 10.255 (2) OF THE DANE COUNTY CODE OF ORDINANCES DOES HEREBY: GRANT the conditional use permit subject to conditions. FOR: Telecommunications tower site. EFFECTIVE DATE OF PERMIT: 4/29/97 EXPIRATION DATE: (See below) THE CONDITIONAL USE SHALL BE LOCATED ON THE PROPERTY DESCRIBED AS FOLLOWS: A parcel of land in the NE 1/4 of Section 14, Town of Blooming Grove described as follows : Commencing at the intersection of the West line of said Northeast quarter and the Southerly right- of-way line of CTH AB; thence N 82° 37' 49" East, along said right-of-way line 149.2 feet to the point of beginning; thence continuing N 82° 37' 49" East, 100 feet; thence S 00° 15' 45" East 575 feet thence West along a line parallel to the South line of said NE 1/4 100 feet; thence N 00° 15 `45" West to the Southerly right-of-way line of County Highway AB and the point of beginning. CONDITIONS: These conditions are specifically made applicable to this permit, any violation of such conditions shall be sufficient grounds for revocation of this permit. 1. The primary use of the communication tower permitted under Conditional Use Permit No. 1400 shall be for the mounting of equipment for Personal Communication Services (PCS) transmissions/receptions and Cellular Telephone equipment. 2. The communication tower placed, constructed or modified under Conditional Use Permit No. 1400 shall accommodate, at a minimum height of 190 feet above grade, the collocation of four additional antenna arrays similar in size and function to that placed on the tower by the holder of said permit. 3. The four collocation sites required hereunder need not be available on the tower as initially placed, constructed, or modified, provided that the tower will support at the specified minimum height the later addition of the required number of collocation sites.
