S-38 <br /> Mr. Ed Moen <br /> 408 Seminole Way <br /> DeForest, WI 53532 <br /> (608) 846-9865 <br /> Dear Mr. Moen: <br /> This letter is to certify that you are authorized by the Wisconsin Department <br /> of Natural Resources to possess, transport, and care for sick or injured <br /> resident wildlife. Threatened or endangered species are not included in this <br /> authorization. <br /> This authority is extended to you as a WILDLIFE REHABILITATION cooperator of <br /> the state pursuant to Sections 23.09, 29.174, 29.175 and 29.02 of the <br /> Wisconsin Statutes. <br /> This permit is issued subject to the following conditions: <br /> 1. The permittee shall keep a written record of all wildlife taken into <br /> possession indicating source and final disposition of animal . <br /> 2. Records and pens are to be open to inspection by authorized agents of <br /> the department. <br /> 3. Wildlife which cannot be rehabilitated are to be turned over to the <br /> local Conservation Warden, Dave Rasmussen, (608) 244-1927. <br /> 4. Rehabilitated animals are to be released to the wild immediately upon <br /> recovery. Very explicity: THESE ANIMALS ARE NOT TO BECOME PETS! <br /> 5. Other disposition is permitted only if approved by the department. <br /> 6. This permit is not transferable and is subject to discretionary <br /> revocation by the department. <br /> 7. This permit must be exhibited to any authorized person upon request. <br /> 8. This permit is effective through December 31, 1985. <br />
