Enclose are two copies of the <br /> updated Conditional Use Permit Application, "Response to Six Standards of a Conditional Use <br /> Permit",with Attachments A and B. Enclosed is a copy of the Mineral Extraction Boundary Map <br /> prepared by Birrenkott Surveying, Inc., a copy of the Site/Operations Plan drawings for Madison <br /> Crushing and Excavating quarry operation area.Also enclosed is a copy of the blasting records for the <br /> facility. <br /> The site/operations plan provides an overall site plan indicating the designated areas of the quarry <br /> operation and indicating surrounding properties within 1,000 feet of the facility. The plan also shows <br /> existing conditions,proposed conditions of the Northern Expansion Area and the propose restoration of <br /> designated areas. <br /> On behalf of Madison Crushing&Excavating Co.,Inc. we request approval of the CUP application. <br /> The information references the existing stormwater management and erosion control plans as well as <br /> the reclamation plan that are on file with both the County and Town of Burke.
