I am <br /> writing to ask you to send a new check for$140.00 made payable to to <br /> Jess Starks, Erosion Control Specialist <br /> Dane County Land and Water Resources Department <br /> 1 Fen Oak Court <br /> Madison, WI 53718 <br /> It is important that you do this so that your Shoreland Erosion Control Permit SE20060233 <br /> will remain in effect. To explain further, your check#618 for$140.00 (copy enclosed) was <br /> placed in the Zoning Division cash drawer in October 2006 with a note for the Erosion Control <br /> Specialist, along with $352.00 in cash. The cash was deposited in late December 2006, as <br /> you can see from the enclosed receipt. <br /> According to a Fax memo in my file, it appears that I contacted you at that time and told you <br /> that we had found the check. Since it was out of date, I asked you to replace it. At the time, <br /> you and I believed that your proposed accessory building would require a zoning permit, so <br /> you agreed to bring the new check to me when you applied for the permit. Further <br /> investigation revealed that no zoning permit is needed for your agricultural use building, so <br /> you did not come to the Zoning office and the check was never replaced. <br /> I apologize for the inconvenience this causes. I am aware that you are in the process of <br /> revising your Shoreland Erosion Control Permit, which expires 03/15/2007. Your cooperation <br /> in replacing the check is required and would be appreciated. <br />
