RE: hest Rid a Addition to Amc , a Subdivision Plat located wit aec on 3, T 7 !I, P. 11 E, Town of Cottage Grove, Dane County, Wisconsin and more particularly regarding Note No. 3 on sheet 1 of said Plat as it applies to Outlot No. 4 on Sheet 1 of said Plat. Plat being recorded in Vol. 49 of Plats on Pages 50 and 51, Document No. 1590878. I, Glen L. Northrop, Registered Land Surveyor, do hereby depose and declare: That I did prepare the above described Subdivision Plat of West Ridge Addition in the year 1977 at the office of ABEX Survey, Ltd. in Cambridge, Wisconsin. That in 1977 and 1978 prior to and during the tine of review and record- ing of this Plat, Wisconsin Administrative Code Rules of Health and Social Services in Code 11-65 were in effect and applicable to the Lots and Outlots created by the above described Subdivision Plat which resulted in the place- ment of Note No. 3 on said Plat. That at this time in June of 1990 the current Wisconsin Administrative Code, ILIIR-35, is in effect and provides for the removal of previously re- quired restrictions in writing by the Wisconsin Department of Industry, Labor and Human Relations per Section IL11R-85.05 of said Code. That an application has been made to the Wisconsin Department of Industry, Labor and Human Relations on behalf of Marl: Steckel, an owner of Outlet 4 of the above described Subdivision flat, for a review of current soil testing data and the on site inspection report conducted by the Dane County Environ- mental Health Section of the Public Health Section of Dane County Department of Human Services. That the Wisconsin Department of Industry, Labor and Human Relations has reviewed and removed in writing, per Section ILHR--05.05, the restriction set forth in Note No. 3 on the Plat as said restriction applies to Outlot No. 4 in said Plat. \€.7% 2 s' (continued on sheet 2) ' � f: "'•••.!•1.0%;..
