RECOMMENDED CONDITIONS <br />Staff has prepared a list M condtions that maybe user! in order to meet the six standards for granting a CUP <br />as listed above to allow the conditional use on the property Please note that the conditions may need to be <br />changed or conditions added to address potential nuisances that may come to light during the public hearing. <br />The following list of conditions is based on a list of conditions that are traditionally used for similar operations in <br />Dane County, customized for this specific site and proposal. <br />POTENTIAL CONDITIONS OF APPROVAL FOR CUP 2405: <br />1) The applicant shall submit an erosion control plan covering the entire CUP area far the duration of <br />operations, and receive ap val of an erosion control d storm water permit (Chapter 15 DCCO) prior to <br />commencing extraction opations. <br />2) The applicant shall apply for and recavle gall other required local state and federal permits. <br />3) The applicant must get a Chapter 30 permit, per ONR regulations, for pumping water as part of the <br />demaledng process. <br />4) The operator shall develop . and operate the site according to the proposed site operations plan. <br />5) Operations shall cease no later than twenty (20) years from the do* of CUP approval. <br />6) ReGpnt shall 0e completed as required! by Chapter 74 of the DCCO and NR135. <br />]) Witness ulMents will need to be established in safe locations and a new Monument Record filed by <br />a Professional Su eyon prior to excavation and disturbance of the existing monument. <br />8) There shall be no blasting, drilling, crushing or screening on the site <br />9) The operator shall maintain the driveway in a dust free manner in accordance with local, state, and <br />federal regulations, and shall clean airy dust or neetl tracked onto public roatls. <br />10) The operator and all haulers shall access the CUP site through the existing access points. <br />11) The access to the driveways shall have gates securely looked when the extraction site is not in <br />operation. The site shall be stoned "NO TRESPASSING.' <br />12) Hours of operation shall be as follows'. <br />a. Mondays through Saturdays 6 am to 6 pm. <br />d. No operations shall take place. on Sundays or legal holidays <br />13) There shall be a safety fence around the entire extraction area at all times. This tense shall be <br />comprised of a basic farce fence with posts and "NO TRESPASSING" signs at regular intervals of at least 100 <br />ff. around the entire penmeter of the anticipated disturbed area. <br />14) The operator shall use water spray bars in the crushing process to reduce dust. Use of spray bars is <br />act required when Me temperature Is below freezing. The operator shall spray the site with water It and when <br />needed t0 control dust. <br />15) There shall be no fuel shred on Site <br />16) There shall be no storage of recycled material on the site <br />1]) There shall be no permanent structures on the site <br />18) All of the material extracted on this site shall be taken to the existing Hellickson pit. <br />19) All vehicles shall be stored on lands three(3)feet above highest water table elevation to prevent <br />against groundwater contamination from Inds or spills. <br />
