SCHEDULE "B" <br />PROVIDER shall be paid on the basis of work completed, when completed at the <br />following rates: <br />a. For the completion of Schedules "A" and "C" provider will be paid $4,950.00 <br />following receipt of invoice. If an addendum to this contract is enacted, the <br />rate shall be as stipulated in the addendum. <br />If PROVIDER is timely with respect to all its obligations under this AGREEMENT, <br />the COUNTY shall make payments due within 30 days of the dates of completion <br />of PROVIDER'S obligations or of billing, as appropriate. If PROVIDER fails to <br />meet time limits, COUNTY'S payments will be delayed an additional 30 days. <br />SCHEDULE"C" <br />Provider shall furnish a draft written report to County staff in both .pdf (portable <br />document format) and Microsoft Word format for review and comment within 10 <br />working days of receiving the documentation listed under Schedule A paragraph <br />1(a -g), and 4(a)(i-ii), or addendum thereto. The written report shall provide all <br />evaluations, determinations, and recommendations listed in schedule A of this <br />contract, or addendum thereto. Provider shall respond to staff questions or <br />concerns regarding the report's compliance with the terms of this contract, or <br />addendum thereto, and shall promptly make any necessary changes. <br />In the event revisions are made to the draft report, Provider shall furnish a copy <br />of the revised draft to County staff for review. Provider shall furnish a final written <br />report to Dane County Planning & Development Staff in both .pdf (portable <br />document format) and Microsoft Word formats providing all evaluations, <br />determinations, and recommendations listed in schedule A of this contract, or <br />addendum thereto. <br />Page 12 of 12 <br />
