Algrem, Justin Engelhart, John D. Schroeter & Rose M. <br />Schroeter are owners). Spouses signatures and middle initials are required to <br />provide valid certificates. <br />11. The required certificates are to be executed. <br />12. As represented, #051104494000 & # 051104493610 are to be legally merged. A <br />copy of the recorded deed is to be provided. <br />13. # 051104496912 is legally required to be included as a lot with this certified <br />survey map. <br />14. If the application refuses to include # 051104496912 as a lot with this certified <br />survey map, a notation is to be affixed to the document "# 051104496912 was <br />created as a separate parcel without the required Dane County certified survey <br />map approval, and currently represents a violation of S.75.17(1)(a), Dane <br />County Code of Ordinances.
