<br />11. <br /> <br />~(1) <br />-Y(!J <br />6. <br /> <br />CUP #1638 <br />PAGE 1 <br /> <br />DANE COUNTY <br />CONDITIONAL USE PERMIT #1638 <br /> <br />THE ZONING AND NATURAL RESOURCES COMMITTEE OF THE DANE COUNTY BOARD <br />PURSUANT TO SECTION 10.255(2) OF THE DANE COUNTY CODE OF ORDINANCES <br />DOES HEREBY: <br /> <br />GRANT: The Conditional Use Permit subject to conditions. <br /> <br />FOR: Mineral Extraction Site. <br /> <br />EFFECTIVE DATE OF PERMIT: 6/27/2000 EXPIRATION DATE: 6/27/2010 <br /> <br />THE CONDITIONAL USE SHALL BE LOCATED ON THE PROPERTY DESCRIBED AS <br />FOLLOWS: <br /> <br />Part of the W 1/2 NE 1/4 and NW 1/4 SE 1/4 Section 2, Town of <br />Vermont described as follows: Commencing at the South quarter <br />corner of said Section, thence North 2615.76 feet to the point of <br />beginning; thence N640 East 730.16 feet; thence NOlO West, 1149.2 <br />feet; thence S790 West, 646.59 feet to the North-South quarter <br />line, thence South 1353.64 feet to the point of beginning. <br />for a distance of 648.91 feet to the point of beginning. <br /> <br />CONDITIONS: <br /> <br />2. <br /> <br />3. <br /> <br />The applicant shall submit an erosion control plan covering the entire CUP area for the <br />duration of operations that meets with the approval of the Dane County Land <br />Conservation Department, and receive approval of an erosion control permit from the <br />Dane County Planning and Development Department. The applicant may commence <br />extraction following approval of the CUP by the ZNR. However, the erosion control <br />permit must be received within 45 days of the approval of the CUP. If the permit is not <br />received within that time, oper~tions must cease until the permit is granted. <br /> <br />The applicant shall apply for and receive all other required local, state and federal permits. <br /> <br />The operator shall develop and operate the site according to the "Proposed Site and <br />Reclamation Plan" dated April, 2000 and the submitted "Operations Statement" dated <br />June 21, 2000 and included in the project file. <br /> <br />Operations shall cease no later than 10 years from the date of CUP approval. <br /> <br />The total disturbed area of the site shall not exceed ten (10) acres. <br /> <br />Reclamation shall be completed within one year after operations have ceased, and shall <br />follow the operator's submitted reclamation plan. Final reclamation of the area must <br />ultimately be up to NR 135 standards. NR 135 is currently pending approval at the state <br />
