<br />POTENTIAL CONDITIONS OF APPROVAL FOR CUP 2141 <br /> <br />1. CUP#2141 will expire (insert number from town action report1 years from the effective date. <br /> <br />2. The applicant shall submit an erosion control plan covering the entire CUP area for the duration of <br />operations. <br /> <br />3. The applicant shall apply for and receive all other required local, state and federal permits. <br /> <br />4. The operator shall develop and operate the site according to the site/operations and plan dated (insert <br />date 1. <br /> <br />5. Hours of operation shall be from 6:00 a.m. to 7:00 p.m. Monday through Friday, and Saturday when <br />necessary from 6:00 a.m. to 2:00 p.m. In instances where DOT project specifications require it, night <br />time operations will be allowed. The permit limits the operation to renter number] night operations per <br />year. <br /> <br />6. The operator shall require all trucks and excavation equipment to have muffler systems that meet or <br />exceed then current industry standards for noise abatement. <br /> <br />7. The operator shall meet DNR" standards for particulate emissions as described in NR 415.075 and NR <br />415.076. <br /> <br />8.
