NEVER ENTER A TREATMENT TANK OR HOLDING TANK UNDER ANY
CIRCUMSTANCE. DEATH MAY RESULT. ESCAPE OR RESCUE FROM THE INTERIOR OF A TANK IS VERY
DIFFICULT.
•
ADDITIONAL INFORMATION:
POWTS INSTALLER POWTS MAINTAINER
Name: STEVEN CROSBY r Name: -.
Phone 849-8771 Phone: SAME
SEPTAGE SERVICING OPERATOR(PUMPER) LOCAL REGULATORY AUTHORITY
Name: Name: MADISON-DANE COUNTY HEALTH
Phone: Phone: 242-6515
This document Is intended to meet minimum requirements of Ch.Comm 83.22(2)(b)(1)(d)&(f)and 83.54(1), (2)&(3),Wisconsin Administrative Code.
Use of this document does not guarantee the performance of the POP/TS.
