NEVER NTER A TREATMENT TANK OR HOLDING TANK UNDER ANY
Ptool CIRCUMSTANCE. DEATH MAY RESULT_ ESCAPE OR RESCUE FROM THE INTERIOR OF A TANK IS VERY
:4' DIFFICULT.
ADDITIONAL INFORMATION:
POWTS INSTALLER POWTS MAINTAINER
•
Name: A,14 2.}.taut . i l,F i t,4 -;rill-z _ Name:
Phone: con?)— P i--81.03 Phone:
SEPTAGE SERVICING OPERATOR(PUMPER) LOCAL REGULATORY AUTHORITY
Name:
Name: Prt.1.1= COLA k(-rY Ptia tc a--L E O L_T
Phone: Phone: Coo S-—k--
to5 15
This document is Intended to meet minimum requirements of Ch.Comm 83.22(2)(b)(1)(d)&(f)and 83.64(1).
