• <br /> Driveway Permits on Town Roads <br /> Standard Operating Procedure <br /> A. Purpose <br /> To provide a standard procedure for issuing driveway access permits to town <br /> roads. This process will outline the responsibilities of the applicant and town employee <br /> involved. Attached is a copy of the Ordinance to be followed. <br /> B. Outline of Process <br /> 1. Person indicates to business office that they would like access to land <br /> from town road. The applicant is given a copy of 1) Standard Operating <br /> Procedure and 2) a copy of the Town Driveway Permit and Specifications <br /> Ordinance. <br /> 2. Payment of$2050.00 is made to the Town Business office. (See page 3 <br /> of the Town Driveway Permits and Specifications Ordinance.) Payment <br /> must be made prior to receiving a building permit. <br /> 3. Person is asked to stake out the area where they would like the access <br /> and call (608-825-8420) when ready for site review. Office notifies <br /> highway personnel applicant is ready. Provide map and other <br /> information as needed. <br /> 4. Inspection of the site is made by one of the highway patrolmen to <br /> determine if the site is appropriate for an access and to determine the <br /> size of culvert. <br /> 5. The highway personnel notify the applicant of the culvert and site <br /> requirements. <br /> 6. BEFORE Driveway is concreted or paved, applicant must have an inspection <br /> to ensure erosion control and 8 inches of fill covers culvert. Applicant must <br /> call 608-825-8420 to request inspection. <br /> 7. The applicant is to call 608-825-8420 when the installation of the culvert <br /> and the driveway is complete. One of the highway personnel will make <br /> an inspection of the site. <br /> 8. The Driveway Permit fowl will be signed at that time, indicating that the <br /> culvert and driveway are correctly installed. Copy is given to applicant. <br /> 9. Applicant is reminded that all CONSTRUCTION EQUIPMENT IS TO <br /> ACCESS LOT FROM THE DRIVEWAY. The intent is to hold owner <br /> responsible for potential damage by construction equipment. <br /> 10. If at the time of the issuance of the occupancy permit by the building <br /> inspector, (and after the highway patrolman has approved it), there is no <br /> damage to the Town Road, and all erosion control has been maintained <br /> throughout the project (ditches and culvert) $2000.00 of the escrowed <br /> money will be returned to applicant. <br /> 11. If in the estimation of the highway patrolman there is damage to the road, <br /> or if proper erosion control has not been maintained, the $2000 escrow <br /> will be retained by the town until all road repairs are made and ditches <br /> and culverts are properly restored. In all instances, $50.00 will be <br /> retained by the Town for administrative fees for permit issuance <br /> and the inspection process. If the cost of repairing damage exceeds <br />
