4. The operator shall develop, operate and reclaim the site according to the revised site plan <br /> dated August 6, 1998 and the operations and reclamation plan dated July 28, 1998. <br /> 5. Operations shall cease and reclamation shall be completed by no later than July 1, 1999. <br /> 6. The operator shall post and maintain at all times a bond or an irrevocable letter of credit in <br /> favor of the Town of Cottage Grove and Dane County in an amount of at least$15,000. <br /> 7. Off-site removal of subsoil overburden shall not be allowed. <br /> 8. All drive aisles shall be either graveled or paved according to the revised operations plans, <br /> and shall be maintained in a dust free manner in accordance with local, state and federal <br /> regulations. <br /> 9. Hours of operation shall be from 6:00 a.m. to 6:00 p.m., Monday through Friday, and from <br /> 8:00 a.m. to 3:00 p.m. on Saturday. Blasting, crushing, screening or washing shall not occur <br /> before 7:00 a.m. on Monday through Friday and 8:00 a.m. on Saturday. No operations of any <br /> kind shall take place on Sundays or legal holidays. Hours may be extended temporarily if <br /> approved by the town board,with written notification given to the County Zoning Division. <br /> 10. Trucks shall exit the site directly to the USH 12 & 18 reconstruction project rather than to <br /> CTH N whenever possible and subject to any required Wisconsin Department of <br /> Transportation approvals. The operator shall work with the Wisconsin Department of <br /> Transportation and highway project contractors to accomplish this goal. Direct access to <br /> USH 12 & 18 shall be prohibited when reconstruction is not occurring in the portion of the <br /> highway project adjacent to the CUP area. <br /> 11. The safety fence shown on the revised plans shall be a minimum of 5 feet in height. The site <br /> shall be visibly signed"no trespassing" at regular intervals around the perimeter of the site. <br /> When the extraction site is not open,the access road onto CTH N shall have gates securely <br /> locked. <br /> 12. All drilling and blasting shall be conducted in such a manner as to have a minimum impact <br /> on surrounding landowners. The applicant shall maintain a list of neighbors who have <br /> requested to be contacted, and shall contact all neighbors on such list by phone prior to any <br /> drilling or blasting. All blasting shall be monitored by seismograph. <br /> 13. There shall be no bulk fuel stored on the site. <br /> 14.No water shall be removed from the site in order to maintain groundwater discharge to <br /> adjacent areas. <br /> 15. The operator shall use water spray bars in the crushing process to reduce dust as necessary. <br /> Use of spray bars is not required when the temperature is below freezing. The operator shall <br /> spray the site with water if and when needed to control dust. <br /> 2 <br />
