Page 1 <br /> DANE COUNTY <br /> CONDITIONAL USE PERMIT #1525 <br /> THE ZONING AND NATURAL RESOURCES COMMITTEE OF THE DANE COUNTY BOARD <br /> PURSUANT TO SECTION 10.255 (2) OF THE DANE COUNTY CODE OF ORDINANCES <br /> DOES HEREBY: <br /> GRANT: The Conditional Use Permit subject to conditions. <br /> FOR: Mineral extraction site. <br /> EFFECTIVE DATE OF PERMIT: 9/28/99 EXPIRATION DATE: (See Below) <br /> THE CONDITIONAL USE SHALL BE LOCATED ON THE PROPERTY DESCRIBED AS <br /> FOLLOWS: <br /> A parcel of land located in the NW 1/4 of the SE 1/4 of Section <br /> 28, Town of Dane, described as follows : Commencing at the East <br /> quarter corner of said Section 28; thence South 608 . 41 feet; <br /> thence West 1919. 64 feet to the point of beginning. Thence <br /> continuing West 454 .22 feet; thence S02°05' 55" East 755.51 feet; <br /> thence East 523 . 66 feet; thence N04°17' 10" West, 203 . 08 feet; <br /> thence N13°55' 15" 190. 44 feet; thence N21°34' 19" West, 226. 00 <br /> feet; thence N15°50' 28" West, 163. 69 feet to the point of <br /> beginning. <br /> conditions: <br /> These conditions are specifically made applicable to this permit, <br /> any violation of such conditions shall be sufficient grounds for <br /> revocation of this permit. <br /> 1. The applicant shall submit an erosion control plan covering the entire CUP area and the new <br /> proposed driveway area for the duration of operations, and receive approval of an erosion control <br /> permit prior to commencing extraction operations. <br /> 2. The applicant shall apply for and receive all other required local, state and federal permits. <br /> 3.The operator shall develop and operate the site according to the submitted site/operations plan. <br /> 4. Operations shall cease no later than three years and two months(38 months, total)from the <br /> date of CUP approval. <br /> 5. Reclamation shall be completed within one year after operations have ceased, and shall follow <br /> the operator's submitted reclamation plan. As each phase is completed, the restoration of that <br /> phase must be completed as much as possible before extracting in the next phase. <br /> 6. To ensure compliance with the reclamation plan, the operator shall post and maintain a bond or <br /> irrevocable letter of credit in favor of Dane County in the amount of$13,485 ($1,500 per acre of <br /> subject site;the subject site is 8.99 acres)until reclamation is satisfactorily completed. <br />
