The operator shall maintain the driveway in a dust free manner in accordance with local, state, <br /> and federal regulations, and shall clean any dust or mud tracked onto public roads. If dust <br /> becomes an issue with neighboring property owners, the applicant shall pave the driveway at its <br /> own expense in a manner that eliminates or significantly lessens the dust problem to the <br /> satisfaction of neighboring property owners. <br /> 8. The operator and all haulers shall access the CUP site only through that point designated as the <br /> entrance/exit on the site/operations plan. All quarry material transport shall be from and to the <br /> north off of and onto CTH P. <br /> 9. The ingress/egress intersection with CTH P shall be located, designed, and constructed to the <br /> satisfaction of the Dane County Highway and Transportation Department. <br /> 10. The access to the driveway shall be sufficiently restricted to prohibit entrance into the <br /> extraction site when it is not in operation. The site shall be signed"NO TRESPASSING." <br /> 11. Hours of operation shall be from 6 a.m. to 4 p.m., Monday through Friday. <br /> 12. There shall be no blasting or drilling on the site. <br /> 13. There shall be a red/orange snow fence signed"DANGER" and "NO TRESSPASSING" in <br /> all areas of potential hazard (e.g. sharp drop-offs) around the perimeter of the site. <br /> 14. The operator shall use spray bars (water) in the crushing process to reduce dust. Use of spray <br /> bars is not required when the temperature is below freezing. The operator shall spray the site <br /> with water if and when needed to control dust. <br /> 15. No water shall be pumped or otherwise removed from the site. <br /> 16. There shall be no bulk fuel stored on site. In cases where the operator wishes to have bulk <br /> fuel stored on site, the operator should indicate why and provide a plan for spill containment that <br /> meets state regulations. The applicant has indicated that they will need to store a diesel fuel tank <br /> on the site for the purpose of fueling a loader to be left on-site. The tank will be a temporary <br /> tank no larger than200 to 300 gallons. The approved tank will be supplied and filled by Danco <br /> Prairie FS. A sand and plastic barrier will be placed beneath the tank to contain any potential <br /> spills. <br /> 17. All excavation equipment, plants, and vehicles shall be fueled, stored, serviced, and repaired <br /> on lands at least three(3)feet above the highest water table elevation to prevent against <br /> groundwater contamination from leaks or spills. <br /> 18. The operator shall require all trucks and excavation equipment to have muffler systems that <br /> meet or exceed then current industry standards for noise abatement. <br /> 19. The operator shall meet DNR standards for particulate emissions as described in NR 415.075 <br /> and NR 415.076. <br /> 20. Dane County and the Town of Dane shall be listed as additional named insureds on the <br /> operator's liability insurance policy, which shall be for a minimum of$1,000,000 combined single <br /> limit coverage per occurrence. The operator shall furnish a copy of a Certificate of Insurance as <br /> evidence of coverage before operations commence. The liability insurance policy shall remain in <br /> effect until reclamation is complete. <br /> 21. The zoning administrator or designee may enter the premises of the operation to inspect those <br /> premises and to ascertain compliance with these conditions or to investigate an alleged violation. <br />
