Explanations for each attachment are provided below. <br /> 1) Completed application form for CUP, including the written statement on how the <br /> proposal meets the 6 standards of a Conditional Use and short text on the form regarding <br /> operations under the CUP. A detailed operations plan is not provided, as it not applicable <br /> to this request. <br /> 2) Scaled map showing neighboring area land uses and zoning districts. This map was <br /> printed from the Dane County GIS website. Please note that the property lines on this <br /> document are not correctly aligned. They are portrayed exactly as provided on the GIS <br /> web site. <br /> 3) Set of plans from the original house construction. These plans are accurate for the <br /> basement area proposed for the dependency living arrangement with the exception of a <br /> wall between the family room and the bedroom #6/craft room. That wall was not built <br /> and the two rooms are open to each other. <br /> 4) A plat of the entire subdivision, showing all parcels, as well as setbacks for each lot. <br /> 5) The full most recent deed of sale, which includes the legal description. <br />
