STATE OF WISCONSIN GRADING, BRIDGE, CULVERT PERMIT <br /> DEPARTMENT OF NATURAL RESO 'CES 3-SC-98-3035LR & 3036LR <br /> Bradt Farms Inc. is hereby granted under S.ctions 30.19(1)(c), 30.12(3) and 30.123, Wisconsin Statutes, <br /> a permit to grade in excess 10,000 sq.ft., .lace a culvert, and place a bridge on the bed and bank of a <br /> tributary to Door Creek, in the Town of ottage Grove, Dane County, in the NW'At of the NE'/ of <br /> Section 8, Township 7 North, Range 11 E.:st, subject to the following conditions: <br /> PERMIT CONDITIONS <br /> 1. You must notify Water Management S pecialist Christopher DeRemer, Madison, WI 53711 (phone <br /> 608-275-3321) before starting constru tion and again not more than 5 days after the project is <br /> complete. <br /> 2. You must complete the project as de•cribed on or before July 1, 2000. You may not begin or <br /> continue construction after this date u ess the Department grants a new permit or permit extension <br /> in writing. <br /> • <br /> 3. This permit does not authorize any wor other than what you specifically describe in your application <br /> and plans, and as modified by the condi ions of this permit. If you wish to alter the project or permit <br /> conditions, you must first obtain writt:n approval of the Department. <br /> 4. You are responsible for obtaining any permit or approval that may be required for your project by <br /> local zoning ordinances or by the U.S. Army Corps of Engineers before starting your project. <br /> 5. You must allow free and unlimited . cess to your project site at any time to any Department <br /> employee who is investigating the proj-ct's construction, operation, or maintenance. <br /> 6. The Department may modify or revok: this permit if the project is not completed according to the <br /> terms of the permit, or if the Departme it determines the activity is detrimental to the public interest. <br /> 7. You must keep a copy of this permit :nd approved plans at the project site at all times until the <br /> project is complete. <br /> 8. Your acceptance of this permit and eff•rts to begin work on this project signify that you have read, <br /> understood and agreed to follow all co,ditions of this permit. <br /> 9. You must restrict the removal of vegetative cover and exposure of bare ground to the minimum <br /> amount necessary for construction. Yo must protect areas of exposed soil from erosion by seeding <br /> and mulching, riprap, sodding, diversic of surface runoff, installation of straw bales or silt screens, <br /> construction of settling basins, or simil.r methods as soon as possible after removal of the original <br /> ground cover and before construction i. completed. Erosion control measures must meet or exceed <br /> the standards found in the Wisconsi Construction Site Best Management Practices handbook. <br /> Erosion control measures must be mai tained until the site is adequately stabilized. <br /> 10. No portion of the bank or upland whic is altered or disturbed and, as a result, unstable may remain <br /> unprotected for more than 7 days. <br /> 11. You and your agent are not allowed to emove the existing vegetative cover or operate construction <br /> equipment within 20 feet of the wate ay. <br />
