Item 7 of <br /> the document indicates "building permits for the structure are on file issued May, 1991." <br /> At a November meeting of the Town Board,you indicated that you would provide the Town <br /> with a copy of a check which woo4d verify payment of building permit fees to the Town of <br /> Middleton. To date, we have nOt received such verification. Please provide a copy of any <br /> verifying documents to my office as soon as possible. <br /> In addition, please provide the following information: <br /> 1)The name, address and telephone number of the general contractor/carpenter who <br /> constructed the accessory building. <br /> 2)The name, address and telephone number of the plumber who installed the plumbing <br /> fixtures and connected the plumbing to the septic system. <br /> 3)The name, address and telephone number of the electrician who installed the electrical <br /> system in the accessory building. <br /> I appreciate your cooperation and prompt attention to this matter. <br /> Cordially, <br /> Grey LaFood <br /> lover!
