The <br /> project site is in Sec . 19, T. 8N. , R. 10E. , Dane County, <br /> Wisconsin. <br /> This work is authorized by a Department of the Army <br /> nationwide permit PROVIDED THE ENCLOSED CONDITIONS ARE FOLLOWED <br /> AND YOU OBTAIN SECTION 401 WATER QUALITY CERTIFICATION from the <br /> Wisconsin Department of Natural Resources (WDNR) . Your project <br /> IS NOT authorized by this nationwide permit until you receive <br /> individual water quality certification or confirmation of pre- <br /> approved water quality certification from WDNR. <br /> There are some specific types of projects for which WDNR has <br /> granted water quality certification. When you contact Mr. Chris <br /> DeRemer of the WDNR office in Fitchburg, WI (phone : (608) 275- <br /> 3321) , you will be informed if your project falls in one of those <br /> categories, or if individual water quality certification is <br /> required. <br /> Please contact the WDNR representative WITHIN 30 DAYS from <br /> the date of this letter to request your certification. WDNR has <br /> asked that we provide you with the enclosed materials and inform <br /> you that if contact has not been made within 30 days water <br /> quality certification will automatically be DENIED WITHOUT <br /> PREJUDICE and your project WILL NOT be authorized. <br /> If water quality certification is granted for your project, <br /> please notify WDNR five days prior to, and again five days <br /> following the discharge . WDNR shall be allowed access to inspect <br /> any discharge for compliance with applicable state laws . <br /> This verification of a nationwide permit is valid for two <br /> years from the date of this letter unless the nationwide permit <br /> is modified, reissued, or revoked. It is your responsibility to <br /> remain informed of changes to the nationwide permit program.
