The plan <br /> includes the following requirements: <br /> 1. The site may only be accessed US Highway 5 lusing a stone tracking pad. The tracking <br /> pad must be a minimum of 50' long x 24' wide x 12"deep and be constructed of 3-inch <br /> clear stone. Any material tracked onto public roads must be removed immediately and the <br /> access controls must be improved prior to resuming construction traffic. <br /> 2. Silt fence must be installed prior to grading in the locations shown on the plan and where <br /> deemed necessary, including down slope of all soil stockpiles. The silt fence must be <br /> maintained until the site has been restored. <br /> 3. Stone check dams must be installed in the locations shown on the plan and where deemed <br /> necessary. Sediment must be removed from the upslope side of the check dams once it <br /> reaches '/2 their height. The check dams may be removed once vegetation has been <br /> established. <br /> 4. The areas identified on the plan must be erosion matted during final site stabilization. <br /> The erosion matting must be WisDOT Class I, Type B as specified in the plan. <br /> 5. Culvert and pipe outfalls must be protected by rock riprap as shown on the plan. The <br /> riprap must be installed such that it forms a bowl-shape with a depth of at least 12"to <br /> dissipate runoff energy. <br /> 6. A financial surety document for 110%of the expected cost of erosion control($9,724) <br /> must be submitted prior to permit issuance. <br /> 7. This project as proposed results in the addition of 19,086 square feet of new impervious <br /> surface to the site. Per s. 14.46(1) DCCO, a stormwater management permit will be <br /> required for any development that results in the cumulative addition of 20,000 square feet <br /> to the site.
