The <br /> plan includes the following requirements: <br /> 1. The site may only be accessed off an existing paved drive via Portage Road. The <br /> construction entrance must be maintained in a condition that prevents tracking of soil onto <br /> public roads. No other site access may be used and material deposited in the road will be <br /> cleaned up throughout and at the end of each workday. <br /> 2. Silt fence must be installed prior to grading in the locations shown on the plan and where <br /> deemed necessary, including the down slope of all soil stockpiles. The silt fence must be <br /> maintained until the site is vegetated and stabilized. <br /> 3. A temporary stone check dam must be installed in the location shown on the plan prior to <br /> any grading within the tributary area. Sediment will be removed from the upslope side of the <br /> check dam once it reaches ' the height of the dam. <br /> 4. The proposed culvert outfall will be protected turf reinforcement mat. The mat must be <br /> installed per manufacturer specifications. <br /> 5. The proposed rain garden and stone crib must be constructed as shown on the approved plan. <br /> Runoff from disturbed areas cannot be directed to the rain garden or rock crib. The rain <br /> garden must be restored after all tributary areas have been stabilized. <br /> 6. Grading must be confined to the areas shown on the plan. No material storage, vehicle <br /> traffic or grading may occur in the areas identified as "undisturbed" or"vegetated buffer" on <br /> the plan. In the event any of these areas are disturbed, additional erosion control measures <br /> will be required. <br /> Land Conservation Division, Lyman F.
