Exhibit #B <br /> Page 2 <br /> ROAD MAINTENANCE: As a part of this application for C.U.P. #643, Madison <br /> Crushing will cooperate with the Town of Burke to upgrade the intersection of <br /> Reiner Road and County Highway T. Madison Crushing will remove the South 50 <br /> feet of Reiner Road up to Highway T and undercut as mutually agreed and <br /> replace the rock as needed at no expense to the Town of Burke. The work will <br /> be completed within 90 working days after the ground thaws in 1990 in <br /> conjunction with the Town of Burke. <br /> The entrance road down into the quarry will be oiled with emulsified road oil <br /> to control dust at all times. <br /> RECLAMATION: When the quarry has been depleted, or within twelve (12) months <br /> of the expiration of the Conditional Use Permit, which ever comes first, <br /> Madison Crushing will slope the banks to not steeper than a one to one slope, <br /> and will cover the flat areas with at least one foot of tillable soil. <br /> Applicant shall comply with all municipal, state and federal laws and <br /> regulations pertaining to reclamation procedures and air and noise pollution <br /> measured at the perimeter of the mineral extraction operation during the term <br /> of this permit. <br /> EXPIRATION: Conditional Use Permit #643 is issued for a five year period. <br />
