GENERAL-SURFACE AND UNDERGROUND (1) The employer shall establish a definite effective program <br /> and continually funcitoning safety program and make every attempt to prevent accidents and increase safety. Employes shall actively participate <br /> in the safety program. <br /> NOTE ITEM CODE <br /> SECTION <br /> Spitz was describing. Also, on May 2, 1989, an attempt was made to <br /> contact Ahlgrimm Explosives in order to inform them of the complaint <br /> and to request copies of their blasters logs for the dates in <br /> question. On May 3, 1989, contact was made with Mr. Rich Bingham, <br /> of Ahlgrimm Explosives and blasters logs were requested. Enclosed <br /> with this report are copies of the blasters logs Mr. Bingham <br /> furnished the department. <br /> On May 3, 1989, contact was made with Mrs. Spitz informing her that <br /> the department had contacted the companies involved and that records <br /> • of blasting operations at the Ziegler Quarry were being requested <br /> and that future blasting operations would be monitored. <br /> On May 8, 1989 blasting resultant ground vibrations were monitored a <br /> the Spitz residence, which is approximately 2,500 feet <br /> South-Southeast of the blasting area. Vibration and Airshock (or <br /> Airbiast) were monitored using the department's Instantel <br /> Seismograph, model DS-377, serial number 0309. In addition to <br /> monitoring ground vibration, the Spitz residence was briefly <br /> examined to view the structural damage reported by in the <br /> complaint. During that examination Mrs. Spitz pointed out several <br /> areas in which gaps were visible between timber members of the <br /> structural framework of the house, as well as one cracked window. <br /> Present during that examination were Mr. Burcalow of Yahara <br /> Materials, and Mr. Ahlgrimm, of Ahlgrimm Explosives Company.
