CUP #989 ' Page 2 CONDITIONAL USE PERMIT #989 CONDITIONS 1. Permittee will provide an adequate security system for the premises and adequate supervision of clients. 2. Clients served shall be limited to women with an AODA (alcohol and other drug abuse) diagnosis in accordance with the DSM IIIR and their children. 3. The number of clients served shall not exceed 56 inpatient, 24 day outpatient and 24 night outpatient. The number of children in the childcare center shall not exceed 19. 4. Access to the facility will be restricted to the present entrance/exit. 5. The sign for the facility shall not exceed thirty square feet; the top of the sign shall not be more than six feet above the ground; and may not be internally lighted. 6. Permittee has agreed that, at the discretion of the Town of Westport or County of Dane, the Women and Children with Special Needs Foundation/Vennebu Center project may be subject to an annual review for each of the next three years. The purpose of said review shall be to examine any unanticipated burdens that the facility may impose upon the community. At the time of the review, it shall be determined: A) . Whether any such burden can be ameliorated without further hearing; B). Whether a full hearing in front of the Town of Westport Plan Commission is warranted. Permittee agrees that if such full hearing is held and it is the finding of the Town of Westport or County of Dane that further restrictions upon the operation of thefacility are reasonable and necessary, such restrictions may be imposed by Dane County. Nothing contained in this condition shall allow the County of Dane to prohibit the original intended use of the facility as a treatment center or to seriously inhibit or undermine the original intended use so that the viability of the project is threatened. a {
