Ms. Helen Hensen 4. <br /> 1. A chlorine residual shall be maintained in the drilling water during the <br /> entire drilling operation. <br /> 2. Notification of the proposed time of grouting shall be given to the <br /> Department of Natural Resources Southern District office at Fitchburg, <br /> telephone number (608) 275-3302, not less than 48 hours prior to the beginning <br /> of the grouting operation. In conducting this grouting, the Halliburton <br /> double or single plug method may only be used if prior approval is obtained in <br /> writing from the Department. <br /> 3. All sampling, reporting and other applicable requirements for both the <br /> construction and operation of the well shall be complied with. These <br /> requirements include the well driller preparing and submitting a construction <br /> report of the well to the Department within 20 days after completion of <br /> drilling the well. For the new well, the construction reporting also requires <br /> that the well driller collect drill cuttings at 5 foot intervals throughout <br /> the depth of the well and at each change in formation. The samples must be <br /> sent to the State Geological Survey for examination and preparation of an <br /> • • accurate geological log of the well. <br /> 4. A sign stating that "WATER FROM THIS WELL SHALL NOT BE USED FOR HUMAN <br /> CONSUMPTION OR FOR THE WASHING OR PREPARATION OF FOOD PRODUCTS" shall be <br /> posted at the site of the well and thereafter maintained in a legible <br /> condition. <br /> 5. Either a pump operation timing device or a direct pumpage measuring meter <br /> shall be installed for well #001 and maintained in good operating condition. <br /> Reports of monthly well pumpage totals (in gallons) for well #001 shall be <br /> made to the Department in accordance with the requirements of s. 144.026 State - <br /> Statutes on the forms that are provided by the Department. . <br /> 6. The Department reserves the authority to require any schedule of <br /> reporting water levels within well #001 that it deems necessary. If a water <br /> level measuring device is not permanently installed on any well that the <br /> Department requires to report water levels, the Department's authority shall <br /> extend to require the well be taken out of service until the reporting can be <br /> conducted. <br /> 7. A free air-break of at least two pipe diameters shall be maintained <br /> between the bottom of the discharge pipe and the high water level of settling <br /> basin. <br /> 8. Upon completion of the aggregate mining operation, the well shall be <br /> permanently abandoned in accordance with s. NR 112.21, Wis. Adm. Code, and a <br /> well abandonment report form or forms shall be completed and submitted to the <br /> Department, unless the owner receives a written approval from the Department <br /> granting approval to continue operation of the well. <br /> 9. The operation of well #001 shall be limited to 400 gpm. with a 12 hour <br /> day pumping cycle followed by a 12 hour day recovery period during which the <br /> well is not pumped at all. <br />
