THIS CARD MUST BE DISPLAYED AT THE BUILDING SITE BEFORE ANY <br /> EARTH DISTURBING ACTIVITY IS STARTED and MUST BE VISIBLE FROM THE ROAD. <br /> IF THE SIGN-OFF CARD IS NOT PRESENT AND PROPERLY DISPLAYED, YOU MAYBE <br /> SUBJECT TO A RECALL FEE.
